daytime & evening phone number, mobile number, fax number, e-mail address,
plus any additional information you think may be helpful.

Please also e-mail a good quality, high resolution (150 res or more), color photo of yourself,
head shot or upper torso "fight pose" only. Other action shots are welcome, as well.
